If period pain stops your life, it is not normal

Endometriosis takes an average of seven to ten years to diagnose worldwide, largely because severe period pain gets normalised. It affects roughly one in ten women and up to half of women with infertility.

Symptoms: severe, progressive period pain; chronic pelvic pain between periods; deep pain during or after intercourse; painful bowel movements or urination, especially around your period; heavy bleeding; profound fatigue; and difficulty conceiving.

Diagnosis: clinical history and examination, transvaginal ultrasound (good for endometriomas, poor for superficial disease), MRI for deep infiltrating disease, and diagnostic laparoscopy — still the definitive test, allowing us to see, stage and treat the disease in one procedure.

Treatment:

• Pain management — NSAIDs, combined hormonal treatment, progestins, hormonal IUD, GnRH analogues with add-back therapy
• Laparoscopic surgery — excision or ablation of endometriotic deposits, removal of endometriomas, division of adhesions
• Fertility treatment — endometriosis affects egg quality, tubal function and implantation. Depending on stage, age and other factors, the route to pregnancy may be surgery, IUI, or straight to IVF. We can offer the whole pathway in one centre rather than sending you elsewhere.
• Long-term management — endometriosis is chronic. Treatment aims to control symptoms and protect fertility, not to promise a one-time cure.
